How to Receive a Payment

If, between July 28, 1987, and February 24, 1998, you (1) were insured by a vehicle casualty insurance policy issued by Defendant State Farm and (2) made a claim for vehicle repairs pursuant to their policy and had non-factory authorized and/or non-OEM (Original Equipment Manufacturer) “crash parts” installed on or specified for their vehicles or else received monetary compensation determined in relation to the cost of such parts, you may be eligible for payment. Please review the table below to determine the actions you should take to receive a payment in this Settlement.

Received Notice of the Settlement in the mail or by email and are NOT a resident of Arkansas or Tennessee No action is needed. You will receive a payment automatically. If you would prefer your payment electronically, please click here. Once the Court approves the Settlement, you will automatically receive a check by mail.
Received Notice of the Settlement in the mail or by email and are a current resident of Arkansas or Tennessee If you lived in a different state at the time you made an eligible State Farm claim for vehicle repairs, you may submit an Arkansas/Tennessee Claim Form seeking a payment. To file an Arkansas/Tennessee Claim Form online, please click here. Your claim must be filed online or postmarked on or before January 31, 2019. You will not receive an automatic payment unless you file an Arkansas/Tennessee Claim Form.
Did not receive Notice of the Settlement in the mail or by email, but believe you are a member of the Class. To file a General Claim Form online, please click here. General Claim Forms must be filed online or postmarked on or before January 31, 2019 You will not receive payment unless you file an eligible General Claim Form.
